I have a few seem to be of Paul Smith shirts and cuffs "optional twin" with the A button and a hole for the button twice.

This is just one element of the design, or you can use with a twin brother?

When I'm using one of the twins, I should have the key?

It is a design feature. Do not remove the keys with the left - the buttons should not be obvious. It is a common understanding of the installation is much less than buying a shirt that is strictly French cuffs.
I like Paul Fredrick shirts myself. With the commitment of the law and the jacket is replaced by a stylish look that will see the ladies.

In general, you can fold the cuff back and want to use the links and hide the button if you do not return to the expense of the folding of the cuff link, and to disrupt and secure with button.
